Get great seats to his favorite sporting event. Arrange for the scoreboard operator to put an "I love you Bill" message up on the scoreboard during the game. Send the team a ball and ask team members to sign and return as a special favor to your fanatic friend.
Invite him to lunch -- at the nearest hotel.
Has he ever had a manicure or pedicure? Why don't you introduce him to one and give one yourself?
